This form merges Sales' Quote Request Sheet (product and order specs) and Operations' Customer Profile Request form (account, contacts, and logistics setup) into a single intake document, so a new account only has to be captured once. Blue section headers were originally Sales' fields, green were originally Operations' fields, and navy sections are shared or new. Per Steve's feedback, a future revision should mirror this form's field structure to how data is entered into Pallet Connect, so information merges cleanly later instead of needing to be remapped.
